[Choir] new link for survey

Hi,

I've heard from a couple of people that they sent the survey but I never
received it.  I have received surveys only from Cecile and Tom.  So if you
are not Cecile or Tom and thought you sent it, try this again with this
link.  Sorry for the bother!
