[Choir] re-opening survey

Hi everyone,

As I mentioned at our zoom rehearsal, the UCM Board has created a survey to
gauge congregants thoughts about re-opening and various mitigation
measures, including vaccines.   It just went out yesterday, but in case you
missed it in your email, you can also find a link to it here:

It doesn't specifically address choir (although it does address small
groups in general, of which choir is one) but there are opportunities to
give additional written comments on each page.
