Thank you, C0C Friends, for noting that I forgot to attach the Planned Giving form. My bad. 

This is the form you would fill out to share with UCM your intentions. Also a reminder that if you’re willing to have your name added to the approximately 20 other UCM members who have informed us of their intention to make a planned gift, we will need to have this form back by April 9th.
